Below is information to complete the onboarding process.

• Background Check Consent

Complete and return to Nicole Moss via email:

• W-9

• Student Protection Policy

• Release of Liability and Assumption of Risk Agreement

• DHEC-1420
  Whether you have previous tuberculosis test results or get a new test, results MUST be on this DHEC-1420 form!
  Suggestion: Broken to Better Urgent Care, 735 Cherry Road, Rock Hill, SC 29732.
  You may submit your receipt for reimbursement to Providence.

You will receive separate email to complete:

• Child Safety Training video and quiz from Ministry Mobilizer
